Lenoir Memorial Hospital dba UNC Lenoir Health Care is giving final notice that Lenoir Memorial Hospital will be destroying medical records related to visits and/or test results performed at Lenoir Memorial Hospital in Kinston, North Carolina from December 31, 1988 and prior. Records selected for destruction will include all records for patients.  Medical records for destruction will begin July 1, 2019.  If any of the above information scheduled for destruction pertains to you and you would like the information provided to you; you must provide UNC Lenoir Health Care Medical Records Department with a signed consent for release of information and photo identification.  You may obtain consent in the Medical Records department or visit www.unclenoir.org and click on the “request medical records” link to download the form.  For additional questions, you may call 252-522-7164.  The Medical Records department is open from 8:00 a.m. to 5:00 p.m. Monday through Friday.
