Advance Directives

As a patient or family member of a patient, it's important to have a conversation around advance care directives—a written statement about how your medical treatment should be handled if you're unable to communicate with your doctor.

Who Helps Plan Advance Directives?

Often, advance directives are referred to as living wills. These documents frequently involve discussions and decisions with your family and loved ones who may help carry out your health care wishes.

Hospital Care Planning Services

The UNC Health Lenoir Information Services Department provides assistance to you and your family to complete advance directives.
