To ensure you receive the best possible care, patient medical information is routinely exchanged between health care providers. This includes medical records sent to other hospitals, specialists, etc.
There is no charge or patient authorization required for this process.
Requesting Medical Records
For personal use, you may request a copy of your UNC Health Lenoir medical record by printing and completing this form.
Once you have completed this form and had it notarized, you will need to submit it to the UNC Health Lenoir Health Information Services Department at:

DESTRUCTION OF MEDICAL RECORDS
Lenoir Memorial Hospital dba UNC Health Lenoir is giving notice that the hospital will be destroying all medical records related to patient visits and/or test results performed at Lenoir Memorial Hospital in Kinston, North Carolina from January 1, 1955, through December 31, 1993. Medical Record destruction will begin January 2, 2025. If you would like a copy of the information provided to you; you must sign an authorization for release of information and provide a form of identification to the UNC Health Lenoir Health Information Services department.
You may obtain a consent in the HIS department or visit www.unclenoir.org and click on the “request medical records” link to download the form. For additional questions, you may call 252-522-7117. The Health Information Services (Medical Records) department is open from 8:00 a.m. to 5:00 p.m. Monday through Friday.
